Just having rewatched this, I have a question: was David just "playing" hard to get? Did he want to have her stew in her own juices (LOL) or was he really having doubts, second thoughts?
In the beginning, when he comes into her office, he seems like the old David, with the twinkle in his eyes. To me, he looks quite happy. Later, as the case goes on, he seems to grow cooler, more detached. Why was that? What did make him have second thoughts (if he had those)? Was there anything Maddie could have said or done to make it easier for him to open himself fully to trying it again?

I think he got cooler and more detached when in the office she made it seem like he should just lay down at her feet and take her back.
Maybe he finally came to the realization of what it would mean getting back with her; this time there would be no running away from a full commitment. He needed time to forgive and forget.
He did seem a teeny bit spiteful when he told her he had to check his calendar.

That's it!!! I do think that he had forgiven her, but he needed time to forget (if that was possible at all). The wound needed time to heal.
But I have to say, in the first office scene she doesn't seem arrogant to me or as if she was taking him for granted. I think she wanted to show her eagerness to be with him, not being embarrassed any more; she wanted to show him that she had changed. He didn't give credit for that, but probably he understood it like you said - that she thought he would fall at her feet too easily.
I dont think he was being spiteful, I think it was his way to tell her - okay,, but not that fast... maybe.
I don't think he wanted to run away from a full commitment (especially not "again", because I don't think he ever ran away from that; she did, but that's another question LOL).

She didn't run away from a commitment but his avoidance of her question on "where was the relationship going" Notice in this episode he tells her the relationship is going somewhere now.

Yes he does - because she behave totally different. She acknowledges him. Notice that he hasnt asked her yet to get married, he hasn't made any commitment like saying we're gonna be together forever yet -- and yet she was okay with it. On the other hand, he had matured very much over those five months. I think they had matured both, quite a lot. They had the best chances to work it out.

Her arrogance came across in her belief that he would be so sure to start things up with her immediately.
